This week at a glance

  1. Discovery playback held with the practice managers; the to-be appointment and rota process was walked end to end and signed off in the room.
  2. Rota supplier confirmed the hourly read cap in writing; the overnight batch window is fixed and the migration stays inside discovery.
  3. Care plan notes profiling complete: 71% of 24 months of historic notes are free text and need a structured summary before they can be reported on.
  4. No RAID items were closed this week; the clinical sign off risk is being held open until the design playback on 12/10.

Risks

RefSubjectLatest updateOwner and due
052180Raised 29/09/2026New Historic appointment extract data qualityCritical 29/09: Profile the extract in discovery, agree a per practice cleansing rule, and migrate 24 months rather than the full history. Robert SindallDue 03/11/2026
052181Raised 29/09/2026New Rota supplier interface rate limitsModerate 29/09: Agree the cap in writing in discovery, and schedule the bulk read overnight inside a single batch window. Robert SindallDue 18/11/2026
052182Raised 29/09/2026New Clinical sign off competes with winter pressuresCritical 29/09: Book the sign off slots in discovery and hold them, with a named deputy clinical lead as a fallback. Robert SindallDue 28/12/2026
052183Raised 29/09/2026New Historic care plan notes are unstructuredModerate 29/09: Report on structured fields only, and leave the free text as a document the clinician reads rather than a figure the report sums. Robert SindallDue 08/12/2026

One further open risk on this project is marked internal and is excluded from this client-facing report.

Decision

  • Both commercial models are presented together. Mars Health Group asked for the fixed price and the time and materials option side by side rather than a single recommendation.
  • Care plan audit uses platform field history. No custom audit object is built; clinical audit reads the platform's own field history on the care plan.
  • Month end lock is a validation rule, not a scheduled job. The lock is enforced at the point of entry rather than by an overnight batch.
